Job summary

CareMed Health Solutions seeks a Wound Care Nurse Practitioner to deliver wound services in long-term care facilities. You will diagnose and treat wounds, prescribe medications, and coordinate care with an interdisciplinary team.

Required are WV NP licensure, board certification, and wound care experience with chronic or complex wounds. Experience with wound vacs and DME is preferred.

Qualifications

• Current, unrestricted NP license in West Virginia.
• Board Certification.
• DEA licensure (Optional).
• Clinical background in adult, family, or geriatric specialties.
• Wound care experience with chronic, infected, or stalled wounds.
• Ability to diagnose and treat diabetic, arterial, venous, or Kennedy ulcers.
• Experience with wound vacs and DME; familiar with advanced wound care products.

Responsibilities

• Provide wound care services for long-term care patients in skilled nursing facilities.
• Prescribe medications, including controlled substances per WV law.
• Order, review, and follow up with treatments and diagnostics.
• Communicate with nursing home staff about changes in patient condition.
• Coordinate with the primary care physician and care plan.
• Collaborate with an interdisciplinary care team to coordinate patient care.

We are currently seeking Wound Care Nurse Practitioners to provide wound services to patients living in long-term care facilities.

WOUND NP ROLE AND RESPONSIBILITIES:

• Provide wound care services for long-term care patients residing in skilled nursing facilities.
• Prescribe appropriate medications, including controlled substances in accordance with WV Law.
• Order, review, and/or follow-up with treatments and diagnostic testing.
• Communicate with the nursing home staff on a regular and as needed to discuss changes in condition.
• and/or the care plan.
• Collaborate with the primary care physician.

WOUND NP QUALIFICATIONS AND EDUCATION REQUIREMENTS:

• Current, unrestricted NP license in the state of West Virginia.
• Board Certification.
• DEA licensure (Optional)
• Clinical background in adult, family, or geriatric specialties.
• Clinical background of wound care; including but not limited to chronic, debilitating, stalled, or infected wounds
• Must be comfortable with diagnosing and treating diabetic, arterial, venous, and mixed etiology wounds or Kennedy Ulcers.
• Must be comfortable staging pressure injuries according to the NPIAP
• Must be familiar with wound care best practice and advanced wound care products; collagens, alginates, foams, silvers, etc.
• Must have experience with wound vacs and other DME, such as pressure relieving/ reducing devices
• Strong preference to having wound certification or be open to getting certified
• Experience working in a long-term care setting.
• Ability to work with an EMR.
• Ability to work within an interdisciplinary care team to coordinate patient care
