Job summary

Vered Healthcare Group is seeking an experienced Physical Therapist for part-time and PRN positions in our Jacksonville facility. The role focuses on providing comprehensive physical therapy to a diverse patient population, designing individualized plans of treatment, and coordinating with other healthcare professionals.

You will evaluate patients, set goals, and document progress while maintaining a commitment to compassionate care and patient independence.

Qualifications

• Active PT license in Florida.
• Experience in outpatient rehabilitation.
• Willing to work part-time and PRN.

Responsibilities

• Perform initial and ongoing PT evaluations per APTA and state standards.
• Develop Plan of Treatment with short and long-term goals.
• Implement and adjust treatment plans based on progress.
• Document patient progress per medical and facility procedures.
• Evaluate mobility, strength, and functional levels; set attainable goals.
• Communicate progress to patients, families, and healthcare teams.
• Plan post-treatment home care or follow-up services.
