• Experience performing PT evaluations to APTA standards.
• Develop and implement Plan of Treatment for patients.
• Coordinate with other healthcare professionals.

Responsibilities

• Perform initial and ongoing PT evaluations per APTA and state standards.
• Develop Plan of Treatment with goals and staff collaboration.
• Set short/long-term goals aligned to patient progress.
• Implement Plan of Treatment on schedule and adjust as needed.
• Document patient progress in medical and facility records.
• Assess mobility, strength, and function; set achievable goals.
• Report progress to patients, caregivers, and staff.
• Plan in-home care after treatment completion.
• Maintain knowledge through workshops and literature.
• Assist staff to ensure a clean, safe rehab environment.
