Job summary

CareMed Health Solutions is seeking a Nurse Practitioner to provide medical services in long-term care facilities in Ohio. The NP will deliver primary and preventative care, manage acute changes, and participate in care planning with an interdisciplinary team.

Responsibilities include prescribing medications per Ohio law, coordinating with nursing staff, and collaborating with the primary care physician to ensure the best resident outcomes. This is a full-time on-site position with benefits.

Qualifications

• Current, unrestricted NP license in OHIO.
• Board Certification.
• DEA licensure.
• Clinical background in adult, family or geriatric specialties.
• Experience working in a long-term care setting.
• Ability to work with an EMR.
• Ability to manage patients with advanced illness and multiple chronic conditions.
• Ability to work within an interdisciplinary care team to coordinate patient care.

• Prescribe appropriate medications, including controlled substances in accordance with OH Law.

• Communicate with the nursing home staff on a regular and as needed to discuss changes in condition and the care plan.
• Collaborate with primary care physician.
