Job summary

We Care Health Centers is seeking a Nurse Practitioner to provide medical services for residents in long-term care facilities. You will deliver primary and preventive care, manage acute changes in condition, and participate in comprehensive care planning within an interdisciplinary team.

Responsibilities include prescribing medications, ordering tests, and coordinating with nursing staff and physicians to ensure quality care for geriatric patients.

Qualifications

• Current, unrestricted NP license in OH state.
• Board certification required.
• DEA licensure.
• Experience in long-term care settings.
• Ability to work with an EMR system.
• Experience caring for adults/geriatric patients.

Responsibilities

We are currently seeking Nurse Practitioners provide medical services to patients living in long term care facilities.

NP ROLE AND RESPONSIBILITIES:

• Provide primary and preventative care for long term care patients residing in skilled nursing facilities.

• Manage Residents with acute change in condition.

• Advanced care planning and chronic care management.

• Prescribe appropriate medications, including controlled substances in accordance with OH Law.

• Order, review and/or follow-up with treatments and diagnostic testing.

• Communicate with the nursing home staff on a regular and as needed to discuss changes in condition.

• and/or the care plan.

• Collaborate with primary care physician.

NP QUALIFICATIONS AND EDUCATION REQUIREMENTS

• Current, unrestricted NP license in the state of OH

• Board Certification.

• DEA licensure.

• Clinical background in adult, family or geriatric specialties.

• Experience working in a long-term care setting.

• Ability to work with an EMR.

• Ability to manage patients with advanced illness and multiple chronic conditions.

• Ability to work within an interdisciplinary care team to coordinate patient care.

• Psych experience a plus
