Job summary

We Care Health Centers is seeking a Nurse Practitioner to provide primary and preventive care for residents in long-term care facilities. The role includes managing acute changes in condition, advanced care planning, and chronic disease management.

The NP must hold an OH license and Board Certification, with DEA licensure. Experience in geriatric care and in an interdisciplinary team is preferred; EMR experience is required.

Qualifications

  • Current, unrestricted NP license in OH
  • Board Certification required
  • DEA licensure required
  • Clinical background in adult, family or geriatric specialties
  • Experience in long-term care setting
  • Ability to work with an EMR
  • Ability to manage patients with advanced illness and multiple chronic conditions
  • Ability to work in an interdisciplinary care team
  • Psych experience a plus

Responsibilities

  • Provide primary and preventative care for long term care patients in skilled nursing facilities
  • Manage residents with acute changes in condition
  • Advanced care planning and chronic care management
  • Prescribe medications, including controlled substances as allowed by OH law
  • Order, review and follow up on treatments and diagnostics
  • Communicate with nursing home staff regularly to discuss changes in condition and care plans
  • Collaborate with primary care physician

We strive to give quality care to residents while working together with the facility to carry out the best care possible. While an interdisciplinary care team to provide medical care to patients in long term care facilities, you will have the opportunity to impact in a positive way the life and wellbeing of the elderly population.

We are currently seeking Nurse Practitioners provide medical services to patients living in long term care facilities.

NP ROLE AND RESPONSIBILITIES:
  • Provide primary and preventative care for long term care patients residing in skilled nursing facilities.

  • Manage Residents with acute change in condition.

  • Advanced care planning and chronic care management.

  • Prescribe appropriate medications, including controlled substances in accordance with OH Law.

  • Order, review and/or follow-up with treatments and diagnostic testing.

  • Communicate with the nursing home staff on a regular and as needed to discuss changes in condition.

  • and/or the care plan.

  • Collaborate with primary care physician.

NP QUALIFICATIONS AND EDUCATION REQUIREMENTS
  • Current, unrestricted NP license in the state of OH

  • Board Certification.

  • DEA licensure.

  • Clinical background in adult, family or geriatric specialties.

  • Experience working in a long-term care setting.

  • Ability to work with an EMR.
