Job summary

CareMed Health Solutions in Ohio seeks a Nurse Practitioner to provide primary and preventive care for residents living in skilled nursing facilities, while monitoring acute changes in condition and supporting advanced care planning. You will collaborate with an interdisciplinary care team, prescribe medications per OH law, order and follow up on tests, and coordinate with primary care physicians.

Ohio NP license, board certification, and DEA licensure are required.

Qualifications

• Current, unrestricted NP license in Ohio.
• Board Certification in Family, Adult, or Gerontological NP preferred.
• DEA licensure required.
• Clinical background in adult, family or geriatric care.
• Experience in long-term care settings.
• Ability to work with an EMR.

Responsibilities

• Provide primary and preventive care for long-term care residents.
• Manage acute changes in condition.
• Participate in advanced care planning and chronic care management.
• Prescribe medications in accordance with Ohio law.

We are currently seeking Nurse Practitioners provide medical services to patients living in long term care facilities.

NP ROLE AND RESPONSIBILITIES

• Provide primary and preventative care for long term care patients residing in skilled nursing facilities.
• Manage Residents with acute change in condition.
• Advanced care planning and chronic care management.
• Prescribe appropriate medications, including controlled substances in accordance with OH Law.
• Order, review and/or follow-up with treatments and diagnostic testing.
• Communicate with the nursing home staff on a regular and as needed to discuss changes in condition.
• and/or the care plan.
• Collaborate with primary care physician.

Np Qualifications And Education Requirements

• Current, unrestricted NP license in the state of Ohio.
• Board Certification.
• DEA licensure.
• Clinical background in adult, family or geriatric specialties.
• Experience working in a long-term care setting.
• Ability to work with an EMR.
• Ability to manage patients with advanced illness and multiple chronic conditions.
• Ability to work within an interdisciplinary care team to coordinate patient care.
