Job summary

We Care Health Centers is seeking a Nurse Practitioner to provide primary and preventive care for residents in long-term care facilities. The role includes managing acute changes, advanced care planning, and coordinating with the interdisciplinary team.

The candidate must hold an unrestricted OH NP license, be board certified, and DEA-licensed, with experience in geriatric or adult care and in long-term care settings. EMR proficiency is required for patient management.

Qualifications

• Current, unrestricted NP license in the state of OH.
• Board Certification.
• DEA licensure.
• Clinical background in adult, family or geriatric specialties.
• Experience working in a long-term care setting.
• Ability to work with an EMR.
• Ability to manage patients with advanced illness and multiple chronic conditions.
• Ability to work within an interdisciplinary care team.

Responsibilities

• Provide primary and preventative care for long term care patients residing in skilled nursing facilities.
• Manage Residents with acute change in condition.
• Advanced care planning and chronic care management.
• Prescribe appropriate medications, including controlled substances in accordance with OH Law.
• Order, review and/or follow-up with treatments and diagnostic testing.
• Communicate with the nursing home staff on a regular and as needed to discuss changes in condition.
• Collaborate with primary care physician.
