Job summary

CareMed Health Solutions in Kentucky is seeking a Nurse Practitioner to provide primary and preventative care to residents in long-term care facilities. You will assess, diagnose, treat, and manage conditions in collaboration with an interdisciplinary team.

Responsibilities include care planning, prescribing medications with DEA compliance, and coordinating testing and treatments for residents with chronic illnesses.

Qualifications

• Current, unrestricted NP license in the state of Ohio.
• Board Certification.
• DEA licensure.
• Clinical background in adult, family or geriatric specialties.
• Experience working in a long-term care setting.
• Ability to work with an EMR.
• Ability to manage patients with advanced illness and multiple chronic conditions.
• Ability to work within an interdisciplinary care team to coordinate patient care.

Responsibilities
