Job summary

Qualifications

• Current PT license in good standing.
• Minimum of one year PT experience, preferably in long-term care.
• Long term care experience is preferred.

Responsibilities

• Plan, develop, organize, implement and direct physical therapy services in a skilled nursing facility.
• Evaluate patients and devise customized rehabilitation plans.
• Maintain accurate documentation and patient records.
• Coordinate with physicians and other practitioners to adjust treatment.
• Instruct and direct aides and assistants to support patient care.
