Job summary

Sixteen Acres Health and Rehabilitation in Massachusetts is seeking Part-time and PRN Physical Therapists to plan, implement, and supervise comprehensive PT services for residents. You will assess patients, develop tailored treatment plans, and coordinate with nursing/therapy teams to optimize function and mobility.

Ideal candidates hold a PT license, have at least 1 year of PT experience (long-term care preferred), and are able to document thoroughly while guiding PT assistants and educating

Qualifications

• Graduate of an accredited school of Physical Therapy; Masters in PT.
• Current PT license in good standing with the State licensing board.
• Licensed Physical Therapist in the state assigned.
• Minimum of one year PT experience; long-term care experience preferred.

Responsibilities

• Plan, develop, organize, implement, evaluate and direct physical therapy services according to regulations.
• Perform evaluations and develop individualized treatment plans.
• Maintain documentation and records; ensure effective communication of PT programs.
• Direct the physical therapy assistant and supervise staff.
• Provide patient education and discharge planning.
