Job summary

Vantage Care Management is seeking a licensed Physical Therapist (PRN) to provide comprehensive PT services in a skilled nursing/long-term care setting. The role involves evaluating patients, developing individualized treatment plans, and coordinating with physicians and care teams.

The PT will document progress, supervise assistants, and ensure adherence to state and federal regulations while promoting quality, patient-centered care. Evening/weekend shifts may be required.

Qualifications

• Graduate of an accredited school of Physical Therapy.
• Master's in Physical Therapy.

Responsibilities

• Plan, develop, organize, implement, evaluate and direct physical therapy services.
• Assess patients and determine PT treatment plans.
• Document progress and ensure HIPAA compliance.
• Direct and supervise PT aides and staff.
• Coordinate with physicians and other care providers.
