Job summary

CommuniCare Rehab, a member of the CommuniCare Family of Companies, has a part-time Physical Therapist (PT) opening at Wyoming Healthcare Center, where our therapists are in-house team members delivering care to residents.

You will evaluate patient charts, screen and assess needs, develop and implement treatment plans, supervise PTAs, and participate in interdisciplinary meetings. Opportunities exist to work at multiple centers, supporting a broad range of patient needs.

Qualifications

• Bachelor’s, Master’s, or Doctorate Degree in Physical Therapy from an Accredited Program.
• Hold a valid or pending State Physical Therapy License

Responsibilities

• Evaluate, Review, and Screen patient’s/resident’s medical charts, evaluations under physician order and develop a treatment plan in accordance with patient’s needs.
• Implement treatment plan through direct treatment, as well as education of the plan of care to all team members, including providing necessary supervision for PTAs.
• Attend/Lead interdisciplinary meetings to relay clinical findings and make recommendations to other members of rehab team, as well as conducting staff/family training and education when appropriate.
