Job summary

CommuniCare Rehab in Pleasant Valley Healthcare Center is seeking a Part Time Physical Therapist (PT) to join our in-house team. You will evaluate residents, develop and implement treatment plans, and educate the care team and families about the plan of care.

As part of the CommuniCare Family of Companies, you’ll have opportunities to work across multiple centers, delivering high-quality, person-centered therapy to residents with complex needs.

Qualifications

• A Bachelor's, Master’s, or Doctorate Degree in Physical Therapy from an Accredited Program.
• Hold a valid or pending State Physical Therapy License.

Responsibilities

• Evaluate, review, and screen patients' medical charts and develop a treatment plan.
• Implement treatment and educate the plan of care to the rehab team, supervising PTAs as needed.
• Lead interdisciplinary meetings to relay clinical findings and recommendations.
• Provide staff and family training; work across multiple centers to meet patient needs.

Job Responsibilities

Job Responsibilities include, but are not limited to: Evaluate, Review, and Screen patient’s/resident’s medical charts, evaluations under physician order and develop a treatment plan in accordance with patient’s needs. Implement treatment plan through direct treatment, as well as education of the plan of care to all team members, including providing necessary supervision for PTAs. Attend/Lead interdisciplinary meetings to relay clinical findings and make recommendations to other members of rehab team, as well as conducting staff/family training and education when appropriate.
