Job summary

Northshore Home Health Care seeks a part-time Physical Therapist to assess, plan, and implement rehabilitative programs to improve mobility, relieve pain, and prevent deformities in patients recovering from disease or injury.

You will perform treatment planning, patient assessment, and documentation, collaborate with physicians, develop discharge plans, educate patients and families, and connect clients with community resources.

Qualifications

• Doctorate, Masters, or Bachelor’s degree in Physical Therapy.
• Current licensure as a Physical Therapist.
• Basic Life Support certification.

Responsibilities

• Assess and plan rehabilitative programs to improve mobility and relieve pain.
• Perform treatment planning, patient assessment, and documentation of care.
