Qualifications

• Must be licensed to practice as a Physical Therapist in the state of employment.
• Must possess a valid state driver's license.
• Automobile liability insurance is required.
• Reliable transportation to visit patients in home settings.
• Ability to drive in all weather conditions.

Responsibilities

• Provide skilled physical therapy to home health patients to improve mobility and function.
• Assess patient needs, develop plans, and implement interventions per physician orders.
• Educate patients and family members on exercises, safety, and home care.
• Coordinate with the branch director and health care team to adjust plans as needed.
