Job summary

Hillcrest Health & Living is seeking a compassionate Physical Therapist to join Hillcrest Rehab Services. You will evaluate patients, develop individualized plans of care, provide skilled treatment, and help patients work toward meaningful functional goals. The position is part-time, approximately 20 hours per week, with Monday–Friday scheduling between 7:00 a.m.

Qualifications

• Graduate from an accredited physical therapy program.
• Active Nebraska PT license.
• BLS certification via AHA.
• Two years PT experience in long-term or similar setting preferred.

Responsibilities

• Evaluate patients and develop individualized plans of care.
• Provide evidence-based interventions to improve mobility and function.
• Document accurately and timely.
• Collaborate with interdisciplinary team.
• Educate patients and families on plans and exercises.
• Coordinate patient transitions within Hillcrest services.
• Ensure safety and infection-control compliance.
