Sentara Health is seeking a Physical Therapist to assess, plan, organize, and participate in rehabilitative programs that improve mobility, relieve pain, and address disabling conditions from disease or injury. • Performs modalities/therapeutic interventions in accordance with accepted standards of care and department policies and procedures.
• Provides effective instruction to the patient and family through standardized educational programs and/or individualized training. Initiate, develop, and maintain collaborative interactions with internal and external team members, patients, and family members to improve patient outcomes.
• Productive and efficient in daily operations. Selects and performs the appropriate assessment procedures.
• Establishes and updates formal plan of care to address individual patient needs.
