Job summary

Enhabit Home Health & Hospice in Illinois is seeking a part-time, salaried Physical Therapist to assess and treat patients with physical impairments at the home health level. You will use modalities and education to improve mobility, independence with daily living, and support families as part of a coordinated care team.

This role includes specialty programs like Balance and Fall Prevention and Total Hip/Knee programs, with supervision by the Branch Director.

Qualifications

• Must possess a valid state driver’s license.
• Must possess automobile liability insurance.
• Must wear seatbelts at all times while driving.
• Must have dependable transportation kept in good working condition.
• Must be able to drive an automobile in all types of weather conditions.
• Must be currently licensed in the state of employment.

Responsibilities

• Examine and treat patients with physical impairments to improve mobility and daily living skills.
• Administer skilled care per physician orders and care plan under supervision of Branch Director.
• Educate families and health care team to support home-based care goals.
• Participate in Specialty Programs such as Balance and Fall Prevention, Spine Safety, and joint replacement programs.
