Job summary

Avamere Rehab of Lebanon is seeking a licensed Physical Therapist for part-time and PRN shifts at our Lebanon, OR location. You will evaluate and treat patients, plan treatment goals, educate families, and document care per professional standards and facility guidelines.

The Associate will supervise assistants, maintain compliance with safety and regulatory guidelines, and participate in continuing education to enhance clinical skills and patient outcomes.

Qualifications

• Graduate of an approved curriculum in Physical Therapy.
• Active CPR/BLS certification.
• Unencumbered licensure to practice.
• Working knowledge of Medicare and payer sources.
• Ability to multitask.

Responsibilities

• Provides physical therapy to patients according to licensure.
• Directs and supervises the work of assistants and rehab staff as applicable.
• Documents evaluations, progress notes, and reports.
• Communicates patient status to care teams and families.
• Participates in continuing education to maintain clinical skills.
