Qualifications

• Licensed physical therapist in good standing.
• Active CPR/BLS certification required.
• Working knowledge of Medicare and payer sources.
• Maintains active state licensure and credentials.

Responsibilities

• Provide physical therapy to patients per licensure.
• Direct and supervise assistants and students as applicable.
• Complete documentation such as evaluations and progress notes.
• Meet or exceed productivity standards.
• Screen individuals who may benefit from therapy.
• Communicate patient status and needs to care team.
