Job summary

Avamere Skilled Advisors, LLC d/b/a Avamere Living in Eugene, OR, is seeking a Physical Therapist to provide and direct all aspects of patient care, including evaluation, treatment planning, and documentation, in a PRN flexible schedule setting.

You will supervise assistants, educate families, ensure safety, and participate in quality assurance, with licensure and CPR/BLS active, Medicare knowledge, and a professional, multitasking approach.

Qualifications

• Graduate of an approved curriculum in Physical Therapy.
• Able to practice unencumbered.
• In good standing with all regulatory agencies and licensing boards
• Must maintain and have an active CPR/BLS during employment.
• Working knowledge of Medicare and other payer sources.
• Full knowledge of residents' rights.
• Exudes professionalism in presentation.
• Ability to multitask.

Responsibilities

• Provides physical therapy to patients according to licensure.
• Directs and supervises the work of assistants, rehab aides, and students as applicable with state and federal guidelines.
• Follows all organizational and facility safety procedures regarding one's personal protection and patient safety.
• Expands and enhances self, in both clinical and professional knowledge through associations and continuing education.
• Maintains appropriate state licensure, certification, and credentialing.
• Submits billing, payroll, and expense documentation in a timely manner.
• Makes appropriate referrals to other members of the rehab team.
