Job summary

Advocate Health is seeking a qualified Physical Therapist for a part-time position in Charlotte, NC. The role involves direct patient therapy, assessment, and care planning in a fast-paced clinical setting.

Responsibilities include evaluating patients, documenting progress, coordinating with physicians and staff, and supervising licensed support personnel. A Bachelor’s degree in Physical Therapy and an active state license are required; BLS is also required.

Qualifications

• Bachelor's degree in Physical Therapy and applicable state license are required.
• BLS certification is required per policy guidelines.

Responsibilities

• Provides direct therapy services to patients seven days a week including holidays.
• Evaluates patients and establishes care plans with modalities and exercise programs.
• Gathers and interprets data from medical records, physician reports, patients, and families.
• Documents evaluations, tests, progress notes, discharge notes, and related charges.
• Communicates evaluation results and progress to physicians and care teams; coordinates schedules and discharge plans.
• Supervises PTA's and Rehab Tech/Aide as required for licensure.
