Job summary

Advocate Health is seeking a dedicated Physical Therapist to join our Rehabilitation Services team at multiple Carolina locations. You will evaluate patients, design and implement evidence-based treatment plans, and coordinate with physicians and staff to support discharge planning.

A Bachelor’s degree in Physical Therapy and an active state license are required, along with BLS. The role offers a 40-hour workweek, weekend coverage, and comprehensive benefits as part of Advocate Health.

Qualifications

• Must have a Bachelor's degree in Physical Therapy.
• Applicable state Physical Therapy license is required.
• BLS certification is required.
• Excellent assessment, documentation, and communication skills.

Responsibilities

• Evaluates patients and develops evidence-based care plans.
• Gathers data from medical records, physician reports, patients, and families.
• Sets functional goals with patients and provides interventions to meet them.
• Documents evaluations, progress notes, and treatment charges.
• Communicates progress to physicians and coordinates schedules and discharge planning.
