Job summary

Advocate Health in Charlotte, NC, is hiring for a full-time, in-home outpatient physical therapist to serve the Northern Charlotte area. The role offers 40 hours/week, Monday–Friday, 8am–6pm (negotiable), with a pay range of $35.50–$53.25 per hour.

This new at-home program delivers clinical care in patients' homes, promoting mobility and independence while reducing transportation barriers. Ideal candidates value autonomy, relationship-based care, and expanding access to rehabilitation services

Qualifications

• Bachelor's degree in Physical Therapy required.
• Active state license in Physical Therapy (NC or eligibility).
• BLS certification required.
• Experience in home health or outpatient settings preferred.
• Ability to work independently with autonomy.

Responsibilities

• Deliver high-quality, one-on-one outpatient physical therapy services in patients' homes, helping improve mobility, function, and independence.
• Provide the same evidence-based outpatient care traditionally offered in a clinic while eliminating barriers to access and transportation.
• Develop individualized treatment plans and collaborate with physicians, caregivers, and interdisciplinary team members to achieve patient goals.
• Utilize technology and flexible scheduling to manage a caseload across a defined geographic area while maintaining strong patient outcomes and satisfaction.
