Job summary

Hanford Post Acute in Hanford, CA is seeking a Part-Time Physical Therapist to evaluate and treat residents promptly, develop individualized plans, and supervise assistants. You will deliver high-quality therapy aligned with physician orders in a post-acute setting and sustain meticulous documentation.

You will collaborate with an interdisciplinary team, participate in care planning, and ensure compliance with PT Board and payer requirements while supporting restorative services and caregiver

Qualifications

• CPR certification and ongoing CEUs as required by the state.
• Bachelor’s degree in Physical Therapy is required.
• Master’s or Doctorate in Physical Therapy is preferred.

Responsibilities

• Evaluate patients within 24 hours or 1 business day of physician referral and document delays.
• Develop individualized treatment plans and obtain physician approval.
• Deliver care as outlined in the treatment plan and supervise PT assistants.
• Communicate progress with the interdisciplinary team and participate in care conferences.
• Maintain timely documentation in accordance with PT Board and payer requirements.
