Qualifications

• DPT or equivalent from an accredited program.
• Current California Physical Therapist license in good standing.

Responsibilities

• Evaluate residents and develop individualized treatment plans.
• Provide skilled physical therapy services to post-acute and long-term care residents.
• Collaborate with physicians, nursing staff, occupational therapists, speech therapists, and families.
• Monitor patient progress and modify treatment plans as needed.
• Maintain accurate and timely documentation.
• Educate residents and caregivers on exercises, safety, mobility, and discharge recommendations.
• Help residents achieve their highest level of independence and quality of life.
