Job summary

HealthDrive is seeking a compassionate Palliative Care Nurse Practitioner for skilled nursing, long-term care, and assisted living facilities. This part-time role (2 days/week) focuses on symptom management, goals-of-care discussions, and family education to improve quality of life.

You will collaborate with facility staff, physicians, and hospice partners, while delivering patient-centered care aligned with residents’ values. Competitive benefits and mentorship are provided.

Qualifications

• Master’s Degree from an accredited college/university.
• State professional Nurse Practitioner license.
• Board Certification Required.
• DEA/Controlled Substance License.
• Valid driver’s license.
• Ability to travel to facilities in assigned territory.
• Excellent written, verbal, interpersonal and organizational skills.
• Ability to use email, internet, and learn healthcare software.

Responsibilities

• Conduct comprehensive palliative care assessments for residents with serious illnesses.
• Evaluate and manage symptoms including pain, dyspnea, anxiety and depression.
• Develop individualized care plans with residents, families, and staff.
• Provide ongoing follow-up visits and monitor treatment effectiveness.
• Coordinate care with primary care providers, hospice, and facility teams.
• Lead goals-of-care conversations and advance care planning.
• Document all care and comply with regulations and quality initiatives.
