Job summary
HealthDrive is seeking a compassionate Palliative Care Nurse Practitioner in a nursing facility setting. This part-time role (2 days per week) focuses on symptom management, care planning, and family discussions to align treatment with patient goals.
You will collaborate with interprofessional teams, coordinate with physicians, and provide education to staff about palliative care principles. Licensure and board certification are required or in progress.
Qualifications
- Master’s degree in nursing or equivalent.
- State NP license in New Jersey.
- Board certification in palliative care or related field recommended.
Responsibilities
- Conduct comprehensive palliative assessments for residents with serious illness.
- Manage symptoms including pain, dyspnea, anxiety, and depression.
- Develop individualized care plans with residents and families.
- Provide follow-up visits and monitor disease progression.
- Order and interpret diagnostic tests as appropriate.
- Collaborate with facility staff and hospice partners.
