Qualifications
- Master’s degree in nursing from an accredited program.
- Current state NP license required.
- Board certification in NP practice required.
- DEA/Controlled Substances license required.
Responsibilities
- Provide comprehensive palliative care assessments for residents with serious illnesses.
- Evaluate and manage symptoms including pain and dyspnea.
- Develop individualized care plans with residents and families.
- Provide ongoing follow‑ups and monitor disease progression.
- Coordinate care with primary care providers and hospice partners.
- Maintain timely clinical documentation in the EMR and ensure regulatory compliance.
