Job summary

HealthDrive Corporation is seeking a compassionate full-time Palliative Care Nurse Practitioner to provide comprehensive palliative services in skilled nursing and long-term care facilities. You will partner with residents, families and interdisciplinary teams to improve quality of life, manage symptoms, and support complex medical decisions.

The role emphasizes patient-centered care aligned with residents' values, goals, and timelines, while reducing hospitalizations and enhancing satisfaction.

Qualifications

• Master's degree in nursing (NP) required.
• State NP license required.
• Board certification required.
• DEA/Controlled Substances license required.
• Valid driver’s license and ability to travel to facilities.

Responsibilities

• Conduct comprehensive palliative care assessments for residents with serious illnesses.
• Evaluate and manage symptoms (pain, dyspnea, anxiety, depression, etc.).
• Develop individualized care plans with residents, families and staff.
• Provide follow-up visits and monitor disease progression.
• Order and review tests and lab results as appropriate.
• Coordinate care with PCPs, specialists, hospice, and facility teams.
• Lead goals-of-care conversations with residents and families.
• Complete advance directives and code status documentation.
• Educate residents, families and staff on palliative care principles.
