Job summary

HealthDrive is seeking a compassionate Palliative Care Nurse Practitioner to deliver on-site, patient-centered care in skilled nursing and long-term care facilities. This part-time role (2 days per week) emphasizes symptom management, goals-of-care discussions, and collaboration with interdisciplinary teams to enhance quality of life for residents.

You will work with residents, families, and facility staff to develop care plans, coordinate services, and educate teams while ensuring compliance

Qualifications

• Master’s degree in nursing or related field required.
• State professional Nurse Practitioner license required.
• Board certification in palliative care or related specialty preferred.
• DEA/CS license required for prescribing controlled substances.
• Valid driver's license with ability to travel to facilities.

Responsibilities

• Conduct comprehensive palliative care assessments for residents with serious illnesses.
• Evaluate and manage symptoms including pain, dyspnea, anxiety, and depression.
• Develop individualized care plans with residents, families, and staff.
• Provide ongoing follow-up visits and monitor progression and treatment effectiveness.
• Coordinate care with primary care providers, hospice, and facility teams.
• Lead goals-of-care conversations and advance care planning.
