HealthDrive is seeking a compassionate Palliative Care Nurse Practitioner to provide on-site services in skilled nursing and long-term care facilities. This full-time role focuses on symptom management, goals-of-care discussions, and interdisciplinary collaboration to improve quality of life for residents.

Qualifications

• Master’s degree in nursing from an accredited program.
• State NP license and board certification required.
• DEA/CS license and valid driver's license required.

Responsibilities

• Conduct comprehensive palliative care assessments for residents.
• Manage symptoms and develop individualized care plans.
• Lead goals-of-care conversations with residents and families.
• Coordinate with primary care providers, specialists, hospice, and facility teams.
• Document in EMR and ensure regulatory compliance.
