Job summary

Qualifications

Responsibilities

• Complete psychosocial assessments for post-hospital care.
• Assist patients in coping with stress related to hospitalization.
• Collaborate with the Clinical Care Management team to determine appropriate level of care.
• Serve as liaison between hospital, patients and Department of Social Services.
• Provide information and referrals to community resources.
