Job summary

Colonial Nursing and Rehab in Crown Point, IN seeks a compassionate Occupational Therapist (OT) to join our skilled nursing and rehabilitation facility. This part-time position focuses on increasing residents’ independence through individualized therapy plans and measurable goals.

You will work with an interdisciplinary team, deliver education to residents and families, and maintain accurate documentation to support high-quality outcomes and compassionate resident care.

Qualifications

• OTR/L license in Indiana required or eligible for licensure.
• Experience in skilled nursing or rehabilitation setting preferred.
• Ability to educate residents and families about techniques and progress.

Responsibilities

• Implement treatment plans developed with the rehab team.
• Conduct evaluations and set measurable goals.
• Educate residents and families on progress and techniques.
• Collaborate with interdisciplinary team to coordinate care.
• Monitor progress and adjust plans as needed.
• Maintain accurate clinical documentation.
