Job summary

Colonial Nursing and Rehab in Crown Point, IN seeks a dedicated Occupational Therapist (OT) to support residents in a skilled nursing and rehabilitation setting.

As a part-time OT, you will implement individualized treatment plans with the rehab team, conduct evaluations, educate residents and families, and monitor progress while maintaining accurate documentation.

Qualifications

• Active OT license (OTR/L) in Indiana or equivalent.
