Job summary

Colonial Nursing and Rehab in Crown Point, IN seeks a compassionate Occupational Therapist (OT) to join our skilled nursing and rehabilitation facility. This part-time position focuses on increasing residents’ independence through individualized therapy plans and measurable goals.

You will work with an interdisciplinary team, deliver education to residents and families, and maintain accurate documentation to support high-quality outcomes and compassionate resident care.

Qualifications

• OTR/L license in Indiana required or eligible for licensure.

Colonial Nursing and Rehab is seeking a compassionate and dedicated Occupational Therapist (OT) to join our skilled nursing and rehabilitation facility in Crown Point, IN. In this role, you will deliver high-quality therapy services designed to improve residents’ independence, function, and overall quality of life.

What You’ll Do

• Implement individualized treatment plans developed in collaboration with the rehabilitation team
• Conduct thorough evaluations and establish measurable short- and long-term therapy goals
• Educate residents, families, and caregivers on techniques that support ongoing progress
• Work closely with interdisciplinary team members to coordinate comprehensive care
• Monitor resident progress and adjust treatment plans as appropriate
• Maintain accurate and timely clinical documentation

What You’ll Bring

• Active Occupational Therapist license (OTR/L) in Indiana or associate’s degree from an accredited OT program
• Commitment to patient-centered care
• Strong communication and interpersonal skills
