Job summary

Colonial Nursing and Rehab is seeking a compassionate Occupational Therapist (OT) to join our skilled nursing and rehabilitation facility in Crown Point, IN. This PRN role focuses on improving residents’ independence and quality of life through personalized therapy plans.

Ideal candidates hold an active Indiana OT license (OTR/L) or equivalent, with strong communication skills and ability to work in an interdisciplinary team.

Responsibilities

• Conduct evaluations and set short- and long-term goals
• Educate residents, families, and caregivers on progress
• Coordinate care with interdisciplinary team
• Monitor progress and adjust plans as needed
• Maintain timely clinical documentation
