Job summary

BAYADA Home Health Care is seeking a part-time Occupational Therapist to perform home health visits in Portage County, OH. You will evaluate patients’ functional status, develop intervention plans, and educate families for independent living.

The role requires an OH OT license and at least one year of OT experience, with preference for prior home health work. You will collaborate with a multidisciplinary team and document via EMR.

Qualifications

• Current OH Occupational Therapist license.
• Minimum one year of recent OT experience.
• Home health experience preferred but not required.
• Ability to work independently and manage time.
• Strong interpersonal and organizational skills; problem solving.
• Proficiency with electronic medical records (EMR).

Responsibilities

• Make home visits to clients in designated geographic territories.
• Perform thorough evaluations of patients’ functional status including muscle function, endurance, visual coordination, communication skills, and ADL/IADL performance.
• Assess cognitive, psychological and motor aspects and develop individualized intervention plans.
• Evaluate home environment for hazards or barriers to independence.
• Identify equipment needs and environmental adaptations.
• Continually assess and revise the OT care plan and collaborate with nursing.
• Educate patients and families in rehabilitative care and activities.
• Document observations and interventions using a state-of-the-art touch pad tablet.
