Job summary

BAYADA Home Health Care seeks an experienced Occupational Therapist to perform home health visits across designated areas. You will evaluate functional status, develop intervention plans, and coordinate with a multidisciplinary team to promote home‑based independence.

The role requires an OH OT license, at least one year of OT experience, and proficiency with EMR. Strong time management and communication skills are essential.

Qualifications

• Current OH OT license is required.
• Minimum of one year OT work experience.
• Graduate of an accredited OT program.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem‑solving skills.
• Solid computer skills; EMR experience preferred.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Perform thorough evaluations of patients’ functional status and ADL/IADL performance.
• Assess cognitive, psychological, and motor aspects and develop intervention plans.
• Evaluate home environment for hazards and necessary adaptations.
• Document observations and interventions accurately using EMR tools.
• Educate patients and families on rehabilitative care and independence.
• Collaborate with nursing and multidisciplinary care teams.
