Job summary

Patriot Homecare is seeking a PRN Occupational Therapist Assistant to serve Trumbull and Portage counties. Under OT supervision, you will provide direct patient care in home settings, document progress, and educate families on exercise programs.

You will assist in evaluation and plan development, and carry out additional duties as assigned. A valid Ohio OTA license and CPR (BLS) certification are required, with at least one year of institutional experience.

Qualifications

• Must be licensed as an Occupational Therapy Assistant in Ohio.
• Accredited OT Assistant program completion.
• Minimum of one year institutional experience.
• Criminal background check required.
• Current CPR (BLS) certification.

Responsibilities

• Provide patient care under the supervision of the OT and delegated tasks.
• Prepare clinical and daily progress notes.
• Instruct patient families and assist in teaching exercise programs.
• Assist in evaluation and development of rehabilitative plan and re‑evaluation as indicated.
• All other duties as assigned.
