Job summary

BAYADA Home Health Care in York, PA is seeking an experienced Occupational Therapist (OT) for a part-time role performing home health visits in senior living communities throughout York County.

The OT will evaluate patients\' functional status, develop individualized intervention plans, educate families, and document care using EMR. Pennsylvania OT license required; at least 1 year of OT experience preferred; strong interpersonal and organizational skills essential.

Qualifications

• PA OT license required.
• Minimum of 1 year OT work experience.
• Graduation from an accredited OT program.
• Ability to work independently.
• Strong interpersonal and organizational skills.
• Solid computer skills; EMR experience preferred.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Evaluate patients’ functional status and ADL/IADL performance.
• Develop individualized intervention plans to achieve outcomes.
• Assess home environments for hazards and adaptations.
• Document observations and interventions using an EMR system.
