Job summary

BAYADA Home Health Care is seeking an experienced Occupational Therapist (OT) for a part-time home health opportunity serving Portage County, Ohio. You will visit adult and geriatric patients in their homes and collaborate with a multidisciplinary team to deliver rehabilitative care.

Qualifications

• Current OH Occupational Therapist license.
• Minimum one year of recent OT experience.
• Graduate of an accredited OT program.
• Ability to work independently and manage time.
• Strong interpersonal and organizational skills.
• Solid computer skills; EMR experience preferred.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Perform thorough evaluations of patients’ functional status (ADL/IADL, cognition, motor).
• Develop individualized intervention plans to achieve favorable outcomes.
• Evaluate home environments for hazards or barriers to independence.
• Identify equipment needs and environmental adaptations.
• Collaborate with nursing in multidisciplinary care planning.
• Educate patients and families on rehabilitation and safety.
• Document observations and interventions using a touch-pad tablet.
