Job summary

Advanced Health Care of St. George is seeking an Occupational Therapist to assess, plan, and implement rehabilitation programs for residents to regain independence in daily living activities.

You will collaborate with an interdisciplinary team, educate residents and families, and stay current with best practices while maintaining licensure and delivering compassionate, resident-centered care in a skilled nursing setting.

Qualifications

• Active Occupational Therapist license required.
• 1+ year experience in a skilled nursing facility or similar setting preferred.
• Ability to build rapport with residents through a compassionate demeanor.
• Excellent interpersonal and communication skills for collaborating with residents, families, and team members.
• Commitment to resident-centered care with empathy and compassion.
• Strong skills in assessment, treatment planning and intervention implementation.

Responsibilities

• Conduct assessments of residents’ physical, cognitive, and psychosocial functioning.
• Develop individualized treatment plans based on assessments, goals, and physician recommendations.
• Provide OT interventions including ADL training, therapeutic exercises, adaptive equipment training, and environmental modifications.
• Document resident progress, treatments, and discharge plans per facility policies and regulatory requirements.
• Collaborate with physicians, nurses, physical therapists, and speech-language pathologists for coordinated care.
• Educate residents and families on techniques, safety measures, and home exercise programs.
• Participate in team meetings and contribute to resident care planning and goal setting.
• Stay current with best practices, evidence-based interventions, and regulatory changes.
• Maintain licensure and adhere to ethical standards.
• Contribute to a positive and supportive work environment through professionalism and teamwork.
• Perform all other duties as assigned.
