Job summary

Evolve Therapy Services is seeking a compassionate Occupational Therapist for an in-house Skilled Nursing Facility in Ponca City, OK. This position is Part Time or PRN, designed for flexible scheduling to support resident care.

You will plan, organize, develop and direct OT services, assess residents, establish treatment goals, coordinate with the interdisciplinary team, oversee COTA, and ensure proper documentation and billing in line with regulations.

Qualifications

• Must hold an Occupational Therapy license in the practicing state.
• OT degree from accredited program.
• Commitment to ongoing licensure and professional development.

Responsibilities

• Screen residents to determine interventions and treatment.
• Evaluate residents and develop short- and long-term goals.
• Communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members.
• Establish individual resident therapy schedules with other team members.
• Deliver therapy per established plan and ensure proper documentation and billing of occupational therapy services.
• Perform all other duties as assigned to effectively treat patients.
• Oversee COTA as needed and ensure documentation and treatments are carried out with clinical excellence and proper reimbursement practices.

Education

Degree in Occupational Therapy

Active Occupational Therapy license in practicing state(s)

Ongoing continuing education and professional development
