Evolve Therapy Services is hiring an Occupational Therapist for an in-house Skilled Nursing Facility in Ponca City, OK. This part-time or PRN position requires direct patient care, plan development, and collaboration with a multidisciplinary team.

Qualifications

• Degree in Occupational Therapy required.
• Active Occupational Therapy license in practicing state(s).
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Screen residents to determine interventions and treatment.
• Evaluate residents and develop short- and long-term goals and plans.
• Communicate evaluation findings and treatment plans to residents and families.
• Schedule resident therapy with team members.
• Provide therapy per plan and document and bill services.
• Perform additional duties to effectively treat patients.
• Oversee COTA and ensure documentation and reimbursement practices are followed.
