Job summary

Evolve Therapy Services in Ponca City, OK is seeking a compassionate Occupational Therapist to lead therapy services in an in-house Skilled Nursing Facility. This role offers part-time or PRN opportunities and requires active licensure.

You will screen residents, develop treatment plans, coordinate with the care team, document care, and oversee COTA as needed to ensure high-quality resident care and compliant reimbursement.

Qualifications

• Degree in Occupational Therapy
• Active Occupational Therapy license in practicing state(s)
• Ongoing continuing education and professional development to maintain current licensure and certification

Responsibilities

• Screen residents to determine best intervention or treatment for each patient
• Evaluate residents to develop treatment plans; establish short- and long-term goals
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members
• Establish individual resident therapy schedules in cooperation with other team members
• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services
• Perform all other duties as assigned and as required to effectively treat patients
• Oversee COTA as needed and ensure all documentation and treatments are carried out with clinical excellence and proper reimbursement practices
