Job summary

Evolve Therapy Services, LLC is seeking a compassionate Occupational Therapist to join our in-house SNF partner in Ponca City, OK. This position is available Part Time or PRN.

You will plan, organize, and direct OT services, screen residents, develop treatment plans, document and bill for services, and supervise COTA as needed.

Must hold OT degree and an active license in the practicing state; ongoing continuing education encouraged; growth opportunities available.

Qualifications

• Must hold an active OT license in the practicing state.
• Experience in skilled nursing or rehab settings preferred.
• Commitment to clinical excellence and compliant documentation.

Responsibilities

• Conduct regular screenings of residents to determine best intervention.
• Evaluate residents to inform and develop treatment plans and goals.
• Interpret and communicate evaluation findings to residents and families and the team.
• Establish resident therapy schedules with other team members.
• Perform therapy according to plan and ensure documentation and billing.
• Oversee COTA as needed and ensure compliance with reimbursement practices.

Job Position

Occupational Therapist for an In-house Skilled Nursing Facility

Job Type

Part Time or PRN

Job Purpose

To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient
• Evaluate residents to inform and develop treatment plans; establish short- and long-term goals and methods to achieve goals
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members
• Establish individual resident therapy schedules in cooperation with other team members
• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services
• Perform all other duties as assigned and as required to effectively treat patients
• Oversee COTA as needed and ensure all documentation and treatments are carried out with clinical excellence and proper reimbursement practices are being followed

Education & Qualifications

• Degree in Occupational Therapy
• MUST have an Active Occupational Therapy license in practicing state(s)
• Ongoing continuing education and professional development to maintain current licensure and certification
