Job summary

Optimal Home Care is seeking a licensed Occupational Therapist to provide in-home OT services in Colorado. You will assess, plan, implement, and monitor treatment plans focused on improving daily living activities and independence for patients in their homes.

Responsibilities include evaluating progress, coordinating with the care team, ensuring HIPAA-compliant documentation, and adapting care plans to patient needs and goals. This role is part-time, on-site in the Denver area.

Qualifications

• Graduate of OT program and Colorado license
• Excellent written and verbal communication
• Ability to document per HIPAA and privacy standards
• Ability to work in home health setting

Responsibilities

• Provide occupational therapy in patients' homes following AOTA standards
• Assess patient needs and goals and adjust interventions accordingly
• Develop and implement Plan of Care in collaboration with the client and MDs
• Collaborate with nursing, therapy, and support staff to coordinate care
• Document evaluations, progress notes, goals, and discharge plans on time and in compliance with regulations
