Job summary

Optimal Home Care is seeking an Occupational Therapist (OT) to provide patient-focused occupational therapy services in the Colorado area. The OT will follow AOTA standards, ensure safety and privacy, and collaborate with a multidisciplinary team to support meaningful daily living activities for clients.

The role requires ACOTE-accredited OT education, Colorado licensure, strong communication and interpersonal skills, and the ability to document and adapt plans of care for home health settings.

Qualifications

• Graduate of an Occupational Therapy program accredited by ACOTE or its predecessor organizations.
• Must be currently licensed and in good standing in Colorado.
• Must be able to communicate with the client language and document client activity.
• Possess effective written and verbal communication skills.
• Strong interpersonal skills.

Responsibilities

• Provides occupational therapy services in compliance with AOTA standards, policies and guidelines.
• Maintains patient safety and privacy in accordance with HIPAA and applicable laws.
• Delivers safe, effective OT services and collaborates with the care team.
• Assesses client needs, sets goals, and documents plans of care.
• Coordinates with other therapies and disciplines to support patient outcomes.
• Develops and implements Plan of Care with client and MD input.
• Monitors and documents outcomes and discharges when goals are met.
